1. Go to https://www.rankonesport.com

2. Click the tab in the top right corner that says “Fill Out Electronic Forms.”

3. Select TX from the dropdown menu.

4. Scroll down and select Seguin ISD.

5. Read over the instructions, then press “Start Online Forms.”

6. Either Create a New Account, Sign In, Search for Account, or skip creating an account and Go To Forms… (although, it is recommended to create an account).

7. Under the Electronic Participation Forms tab – be sure to fill out BOTH the “Seguin ISD Athletic Policies”, “UIL Forms Signature”, “Student Drug Testing Consent”, AND “Student Athelte Info Page” → press SUBMIT at the bottom of the page after completing each form

***If you get an error message that says the student ID # doesn’t match the name – try adding a zero in front of the student ID # - if that does not work, please e-mail me at seanderson@seguin.k12.tx.us/calvarado@seguin.k12.tx.us/

8. To be sure you submitted the online paperwork correctly, please check the e-mail address you provided for confirmation. If you did not get a confirmation e-mail, forms were not submitted correctly and should be filled out again. 

*Please fill out these forms as soon as possible as your athlete may not participate in any sport until all necessary paperwork is on file with the athletic trainer. 
Also, please only submit these forms online – Do NOT PRINT them out and turn them in.
 
** ALL athletes MUST have an updated Physical and a current Medical History Form on file with the athletic trainer.

Required Items:

In order for your athlete to participate in athletics, cheer, dance, band, folklorico or ROTC, the UIL and Seguin ISD require an updated physical exam and medical history EVERY SCHOOL YEAR. These physical exams are to be completed by a Physician only!

We will accept physicals for the academic year, physician dated, on or after April 1.

In addition to a physical, Seguin ISD also requires parents and student-athletes to fill out several online forms through Rank One @ www.rankonesports.com

Each student-athlete must be cleared by a physician & the Athletic Trainer prior to participation at the high school level.

All required forms must be completed in their entirety and returned to the athlete’s head coach or Athletic Trainer before a student participates in any try-out, practice, athletic class, open gym, open weight room, athletic competition,  or travels with an athletic team for any purpose.

The student-athlete is required to use the UIL Pre-Participation-Physical Examination form (which can be found on RankOne or the Seguin ISD Athletics’ Website). NO OTHER physical examination form can be accepted as per UIL. A new physical exam must be given prior to each school calendar year as per district rules.

PPE Form must be completed by either a Physician, a Physician Assistant licensed by a State Board of Physician Assistant Examiners, a Registered Nurse recognized as an Advanced Practice Nurse by the Board of Nurse  Examiners.
