* A physical is required for all incoming 7th grade students who want to participate in athletics as well as any athlete who has never had one on file with Seguin ISD.

* Any 8-12 grade student-athlete with a current physical on file will need to complete the Medical History Form before participating. If questions 1-6 of the form are marked "yes" then the student must have a new physical.

* Physicals will be offered for $25 at Seguin High School on July 22 and Aug. 26 from 5:30-7 p.m. Please enter the middle set of doors at the Seguin ISD Athletics Department on the south end of Seguin High School.

To adhere to the safety guidelines, we are asking that only the student-athlete and one parent enter the facility. Everyone must wear a mask and will be screened upon entry. If you do not feel well please do attend.
